Payment Policy
Thank you for choosing us as your provider. We are committed to providing you with quality and affordable health care. Because some of our patients have had questions regarding patient responsibility for services rendered, we have been advised to develop this payment policy. Please read it, ask us any questions you may have, and sign in the space provided. A copy will be provided to you upon request.
Required forms: Please DOWNLOAD the following forms and bring them with you completed.
Payments: All payments must be paid at the time of service via Venmo, @merissa-turner-np.
Insurance: We currently do not participate with any insurance companies. We can provide a superbill to you if you would like to file that with your insurance. Please provide request in writing to our office.

Fees are as follows:
- Medical cannabis evaluation, initial $100
- Medical cannabis evaluation, renewal $75
- Urgent and Primary care appointment $70
- Lost certificate for medical cannabis reprint $15
- ADHD and mental health evaluation, initial $175
- ADHD follow up $80
- Medication refill $35
- In person appointments $200
- CCSO employees – negotiated rate
Missed appointments. Our policy is to charge for missed appointments not canceled within
with-in 24 hours before the appointment time. These charges will be your responsibility and
billed directly to you. Please help us to serve you better by keeping your regularly scheduled
appointment.
Nonpayment. If your account is over 30 days past due, you will receive a letter stating that you
have 20 days to pay your account in full. Partial payments will not be accepted unless otherwise
negotiated. Please be aware that if a balance remains unpaid, we may refer your account to a
collection agency and you and your immediate family members may be discharged from this
practice. If this is to occur, you will be notified by regular and certified mail that you have 30
days to find alternative medical care. During that 30-day period, our physician will only be able
to treat you on an emergency basis.
Appointments. Appointments are required for evaluation and subsequent prescribing of
controlled substances. There is an initial intake appointment that must be attended virtually,
cost is $175. Appointments can be spaced to every 3 months once medication management is
stable, these appointments must be attended virtually, cost is $80. Months that don’t require a
virtual appointment have a prescription refill fee of $35. Intake forms are required for each
type of appointment or refill, they must be completed prior to the appointment or before
medications will be refilled. There is an in person appointment requirement by the DEA once
every two years, those appointment fees are $200.
